How clinics use OPD slips

Reception fills the slip, takes the vitals and hands the slip in with the patient. The doctor writes on the prescription pad, and reception tears off the receipt when the fee is paid. Number receipts in order, so a missing number shows a receipt that was never written.

For a printed prescription pad with the same letterhead, use the prescription pad maker.

Questions

What is an OPD slip?

An OPD slip is the sheet a clinic fills when an outpatient registers: OP number, date, name, age, sex, phone, complaint and vitals. It goes to the doctor with the patient, so the doctor sees who is next and why they came.

What should a doctor fee receipt show?

The clinic’s name and address, the doctor’s name and registration number, a receipt number, the date, the patient’s name, what the fee was for, the amount, how it was paid, and a signature.

Can I print it on half an A4 sheet?

Yes. The slip and the receipt each take half of one A4 sheet. Print on A4 and cut along the dashed line.
