OPD register format

A landscape A4 register with the columns a clinic fills for every outpatient visit, 18 rows to a page. Type your clinic name to put it on the header, then print as many pages as you need or download it for Excel.

How to use this register

Write one row per visit, in the order patients arrive. Use the S. No. column as the day’s token, and start each new month on a fresh page, so the month’s total is the last S. No. on its last page.

Where the Clinical Establishments Act applies, its minimum standard for clinics says “Every patient visiting the clinic shall be registered” and asks for OPD records to be kept as per medical council guidelines (standard CEA/Clinic-011, clauses 10.1.1 and 9.1). A register like this one, filled for every visit, meets the first part on paper.

Write the fee collected in the Fee column as each patient leaves. At closing time the column total should match the cash and UPI in hand, which is the quickest daily check a small clinic can make.

Looking for a patient in a paper register means turning pages. In MyClinicDesk the same register fills itself from each visit, and a search by name, phone or OP number finds every visit in a second. See OPD software for small clinics.

Questions

What columns should an OPD register have?

At minimum: a serial number, the date, the OP number, the patient’s name, age and sex, a phone number, the complaint or diagnosis, the treatment or advice, and the fee collected. A column for new or review visits lets you count first visits separately at the end of the month.

Is there an OPD register format in Excel?

Yes. The Download for Excel button saves the same columns as a CSV file with 100 numbered rows, which opens in Excel and Google Sheets.

How do I keep the OP number the same for a returning patient?

Give each patient one OP number at the first visit and write that same number on every later visit, marking the visit as Review. Searching an old register by OP number then finds all of that patient’s visits.

An OPD register that fills itself

MyClinicDesk gives each patient an OP number, records every visit and the fee paid, and keeps each patient’s outstanding balance.
