The Hidden Hours: How Much Time a Paper Clinic Spends on Admin Each Week

MyClinicDesk Team··4 min read

Ask a doctor how much time the clinic spends on paperwork, and the doctor says "some". Nobody at a small clinic measures it. The time is in small pieces: two minutes to find a card, five minutes to write a bill, ten minutes of reminder calls.

These small pieces add up to hours every week. This guide shows you how to measure those hours at your own clinic.

What studies say about admin time

The best-known study of admin time comes from the United States. Sinsky and colleagues watched 57 doctors at work in their offices. The doctors spent 27.0% of their office time face to face with patients.

For every hour with patients, those doctors spent nearly two more hours on records and desk work. They also did one to two hours of this work at home each night.

Indian clinics are different. Consultations are shorter, and the front desk does more of the paperwork. But the study shows a pattern that every clinic knows: admin work grows quietly until it takes more time than the patients.

Registration alone takes time. A small study at an Indian tertiary hospital timed registration at the counter. Offline registration took 10.37 minutes per patient on average. The same study found that online registration with ABHA was much faster.

Measure your own clinic in one week

You do not need a study. You need one week and a sheet of paper at the desk. Ask your receptionist to note the minutes for each job below:

  1. Registration of new patients, by hand.
  2. The search for old cards and files.
  3. Phone calls about timings, fees and slots.
  4. Reminder calls for tomorrow's appointments.
  5. Follow-up calls to patients who must come back.
  6. Bills and receipts, written by hand.
  7. Calls and notes about dues.
  8. The end-of-day cash and UPI count.

At the end of the week, add the minutes for each job. Most clinics are surprised by the total for jobs 2, 3 and 4.

An example with your own numbers

Here is how to turn the sheet into hours. The numbers in this example are examples only. Use the numbers from your own sheet.

Suppose your clinic registers 10 new patients a day. Suppose each registration by hand takes 6 minutes. Then registration takes 60 minutes a day.

Suppose the desk makes 15 reminder calls a day at 2 minutes each. Then reminder calls take 30 minutes a day.

Over six working days, those two jobs take 9 hours a week. That time is more than one full working day of your receptionist.

Which hours a system can remove

A simple system removes the repeat jobs. It cannot remove the jobs that need a person, such as a talk with a worried patient.

This is how MyClinicDesk changes the jobs on your sheet:

  • Registration: patients scan a QR code at the desk and fill the form on their own phone. Staff check the record.
  • Old cards: staff search by name, phone or OP number.
  • Phone calls about slots: the WhatsApp front desk answers booking messages at any hour.
  • Reminder calls: every appointment sends WhatsApp reminders 24 hours and 3 hours before.
  • Follow-up calls: the patient gets a WhatsApp reminder on the follow-up date.
  • Bills: staff make the bill from the price list and send it on WhatsApp.
  • Dues: each record shows the balance due.
  • The day's count: the reports show what the clinic billed and collected.

What to do with the hours

The hours that the system frees go back to the patients at the desk. Your receptionist has time to greet each patient and keep the queue calm. The doctor gets fewer interruptions about lost cards and missed calls.
